Job Description:
Hospice Registered Nurse (RN) Case Manager
Full-Time MondayFriday 8a5p

As a Hospice RN Case Manager youll guide patients and families through lifes most meaningful moments. Youll coordinate compassionate patient-centered care and lead a team of dedicated professionals who make a true difference every day.

What Youll Do
Conduct comprehensive patient assessments and create personalized care plans
Collaborate with the interdisciplinary hospice team to provide high-quality care
Educate and support patients families and caregivers on symptom management pain control and hospice philosophy
Monitor patient condition and communicate changes to the care team and physician
Offer emotional and spiritual support at end-of-life
Provide care in home settings assisted living and skilled nursing facilities
What Youll Need
Graduate of an accredited nursing program (Diploma ADN or BSN)
Active RN license
1 year of clinical experience (hospice or home health preferred)
Reliable transportation valid drivers license and current auto insurance
Physical ability to sit stand bend lift and move intermittently (up to 50100 lbs)
