Summary:
The Care Coordinator works interdependently with other health care professionals to provide quality health care. Beyond the basics of the traditional medical assistant role the Care Coordinator is responsible for guiding physicians and advanced practice providers through the clinic schedule to maximize their ability to care for their patients in a connected way. They ensure that the patient feels comfortable confident and satisfied with their visit. They prime the provider to be informed and prepared upon entering each visit. They coordinate next steps and follow-up after the visit.
