Key responsibilities

Manage and deliver hospice patient care plans

Support patients and families within the assigned area

Complete all charting and documentation accurately and on time

Manage time and scheduling independently
Schedule

Saturday and Sunday 8:00 a.m. to 8:00 p.m.

One additional day (Friday or Monday) from 8:00 a.m. to 5:00 p.m.

Candidates available only for Saturday and Sunday may also be considered
Travel
Qualifications
Required

Associate degree or higher in Nursing

At least 2 years of hospice experience

Active Registered Nurse (RN) license in Pennsylvania

Valid drivers license reliable transportation and current auto insurance
