Job Summary

Duties:

Monitors patients for and provides education on food/drug interactions.
Records food and drug interactions in the education section. Consults with physicians regarding appropriate diet modification.
Screens and assesses patients in an accurate and timely manner based on levels of care; determines patients nutritional needs and status and provides follow-up care.
Interviews patients and/or their families to obtain history of food habits and preferences.
Interprets pertinent laboratory physical and social data in the medical record as it applies to nutrition care.
Provides nutrition education and records patients diet and diet instructions and the patients understanding of the diet in the education section.
Modifies patient menus as needed.

Skills:

Minimal one (1) year of experience
Two (2) years of experience.- preferred

Education:

Graduate from accredited college/university with a degree in Dietetics or Foods and Nutrition. - required
Masters degree - preferred

Required Certifications & Licensure:

For post-acute locations requirements New York State Certification issued by the New York State Education Department (NYSED) as a Dietitian-Nutritionist.
For Acute locations: Registration by the Commissions on Dietetic Registration (CDR) as a Registered Dietitian Primary Source Verification - Required.

Preferred Certifications & Licensure:

AHA BLS.
Special Note for Dietitians assigned to Post-Acute Care (Long-Term Care) facility: Under the Medicare and Medicaid Programs Reform of Requirements for Long-Term Care Facilities as outlined in Section 483.60 of Title 42 of the Code of Federal Regulations all newly-hired Dietitians assigned to a Post-Acute Care (Long-Term Care) facility are required to have New York State Certification issued by the New York State Education Department (NYSED) as a Dietitian-Nutritionist at the time of hire.
