About the MyLife Project
The MyLife team empowers the voices of individuals and supports positive change recognising that children young people and families are the experts in their own lives.
As a multidisciplinary team we provide seamless support across pathways delivering individualised interventions funded primarily through Targeted Care Packages. These packages are designed to help young people transition from or avoid entry into residential services.
The Role Case Manager
As a Case Manager with Anchors MyLife team youll play a pivotal role in empowering young people to achieve their goals and live sustainable independent lives. Working within a trauma-informed client-centred framework you will provide oversight guidance and practical support in collaboration with Youth Workers families and other care team members.
