LOCATION: Nanuet NY
SALARY: $33.00 per hour
HOURS: Monday - Friday (40 hours a week)
We are seeking a Licensed Practical Nurse (LPN) to join our Assertive Community Treatment (ACT) Team in Rockland County. This role provides essential psychiatric and medical support health education and medication management to individuals living with serious mental illness and co-occurring substance use disorders.
Working under the supervision of the ACT Registered Nurse and Psychiatrist/MD the LPN will deliver care in participants homes and community settings monitor health assist with medications and support the interdisciplinary team in treatment planning and care coordination.
Responsible for on-call shifts monitoring and responding to after-hours calls to the program emergency phone. Shifts are for one week at a time on a rotating basis.
Qualifications:
Current NYS LPN license and registration required
Valid NYS drivers license required
Experience in mental health or psychiatric nursing preferred
Strong communication organizational and teamwork skills
Bilingual (Spanish Haitian Creole or Yiddish) a plus
