Minimum Qualifications:

Preferred Qualifications:
- Masters degree in Social Work or a related field.
- Certification in case management or a related specialty.
- Experience working with child/family welfare programs or in program development.
Responsibilities:
- Conduct thorough assessments of clients needs and circumstances to develop personalized care plans.
- Coordinate and monitor services provided to clients ensuring they receive appropriate support and resources.
- Facilitate communication between clients families and service providers to promote collaboration and understanding.
- Implement crisis intervention strategies as needed to address urgent client situations effectively.
- Evaluate and adjust care plans regularly based on client progress and changing needs.
