The Case Manager plays a crucial role in the Care Services industry by coordinating comprehensive care for clients facing various challenges. This position involves assessing client needs developing individualized care plans and ensuring that clients receive the necessary services and support. The Case Manager will work closely with clients families and multidisciplinary teams to facilitate effective communication and collaboration. A key outcome of this role is to empower clients to achieve their goals and improve their overall well-being. Ultimately the Case Manager is dedicated to enhancing the quality of life for individuals by providing tailored support and resources.

Minimum Qualifications:

  • Bachelors degree in Social Work Psychology or a related field.
  • Must be fluent in Spanish
  • Experience in case management or social services particularly in a health care setting.
  • Strong understanding of mental health issues and crisis intervention techniques.

Preferred Qualifications:

  • Masters degree in Social Work or a related field.
  • Certification in case management or a related specialty.
  • Experience working with child/family welfare programs or in program development.

Responsibilities:

  • Conduct thorough assessments of clients needs and circumstances to develop personalized care plans.
  • Coordinate and monitor services provided to clients ensuring they receive appropriate support and resources.
  • Facilitate communication between clients families and service providers to promote collaboration and understanding.
  • Implement crisis intervention strategies as needed to address urgent client situations effectively.
  • Evaluate and adjust care plans regularly based on client progress and changing needs.

Skills:

The required skills in case management and social services are essential for effectively assessing client needs and developing tailored care plans. Crisis intervention skills are utilized daily to address urgent situations ensuring clients receive immediate support. Knowledge of mental health and human services allows the Case Manager to navigate complex client situations and advocate for necessary resources.
