At Anchor we work alongside children families and individuals in Melbournes outer east to create safe and stable homes. Were seeking a compassionate Case Manager to join our Kinship Care team supporting children who cannot live with their parents and are cared for by extended family or close networks.

About the Role

As a Case Manager you will work closely with children young people and kinship carers to provide high-quality assessment support and case management. This is a pivotal role in ensuring children and young people experience stability safety and care within their extended family networks.

You will work from a trauma-informed strengths based approach centering the voice of the child and carer in everything you do. This is an opportunity to work within a passionate team and help shape the delivery and future development of our home based care services.

Key Responsibilities

  • Provide quality placement assessment case management and support services to children and young people in kinship care.
  • Conduct thorough and ongoing client assessments to inform service planning and intervention.
  • Monitor client wellbeing implement appropriate interventions and evaluate outcomes.
  • Coordinate care and liaise with professionals families carers and other stakeholders.
  • Engage in assertive outreach when appropriate to promote safety and stability.
  • Participate in carer reviews support meetings and statutory planning processes.
  • Work collaboratively with the broader care team to deliver high quality services.
  • Contribute to service planning development and continuous improvement efforts.

About You

  • A relevant tertiary qualification in Social Work Psychology Community Services or a related field.
  • Experience in Out of Home Care child protection family services or related areas.
  • Ability to build strong respectful relationships with children families carers and professionals.
  • Strong case management skills including planning coordination and documentation.
  • Current Victorian drivers licence and Working with Children Check.
