Overview
We are seeking a compassionate and skilled Registered Nurse (Home Health) to join our clients expanding team in Lincolnshire Illinois. This position is ideal for a dedicated RN who thrives in an autonomous environment and values providing personalized one-on-one care to patients in their homes. The Home Health RN will play a vital role in promoting patient independence managing complex conditions and supporting patients through recovery and ongoing health management.
Key Responsibilities
Conduct comprehensive assessments and provide skilled nursing care in patients homes
Develop and implement individualized care plans in collaboration with physicians and interdisciplinary care teams
Educate patients and family members on disease management medications and follow-up care instructions
Monitor and document patient progress accurately and in compliance with healthcare regulations
Communicate effectively with physicians case managers and other care providers to ensure continuity of care
Support the emotional and physical well-being of patients and families through compassionate and professional care
Schedule and Shift Details
Monday through Friday day shift
One weekend per month
Two to three on-call days per month
Two holidays per year
This is a Pay Per Visit position
Travel
Local travel required within the Lincolnshire area (travel reimbursement provided)
Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Illinois
Minimum of 1 year of professional RN experience (home health experience preferred)
Current BLS or ACLS certification through the American Heart Association
Valid drivers license reliable vehicle and current auto insurance
Preferred
Previous experience in home health hospice or community-based care
Excellent communication assessment and critical-thinking skills
