Overview
We are seeking a compassionate and skilled Registered Nurse (Home Health) to join our clients expanding team in Riverside this role you will deliver professional nursing care to patients in the comfort of their own homes promoting recovery independence and overall well-being. The ideal candidate is a motivated organized clinician with strong communication skills and a passion for home-based patient care.

Key Responsibilities
Conduct comprehensive assessments and provide skilled nursing care in patients homes
Develop and update individualized care plans in coordination with physicians and interdisciplinary team members
Educate patients and families on treatment plans medications and disease management
Monitor and document patient progress adjusting care plans as necessary
Ensure compliance with all clinical and regulatory standards through accurate and timely documentation
Collaborate effectively with the healthcare team to ensure high-quality continuous patient care

Schedule and Shift Details
Monday through Friday day shift
1 weekend per month and 2 3 on-call days per month
2 holidays per year
This is a Pay Per Visit position

Travel
Local travel required within the Riverside area (travel reimbursement provided)

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Illinois
Minimum of 1 year of professional RN experience (Home Health experience preferred)
Current BLS or ACLS certification through the American Heart Association
Valid drivers license reliable vehicle and current auto insurance

Preferred
Prior experience in home health or community-based nursing
Excellent clinical communication and time management skills
