Overview
We are seeking a compassionate and experienced Registered Nurse (Home Health) to join our clients growing team in Wadsworth Illinois. This position focuses on providing high-quality nursing care to patients in their homes supporting recovery independence and overall well-being. The ideal candidate will demonstrate clinical excellence strong communication skills and a genuine passion for patient care.

Key Responsibilities
Conduct comprehensive patient assessments and provide skilled nursing care in home settings
Develop implement and evaluate individualized care plans in coordination with physicians and interdisciplinary teams
Educate patients and families about health management medications and post-care procedures
Monitor and document patient progress while ensuring compliance with regulatory standards
Collaborate effectively with other healthcare professionals to ensure continuity and quality of care

Schedule and Shift Details
Monday through Friday day shift
One weekend per month and 2 3 on-call days per month
Two holidays per year
Flexible scheduling and autonomy in daily workflow
This is a Pay Per Visit position

Travel
Local travel within the Wadsworth area required (mileage reimbursement provided)

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Illinois
Minimum of 1 year of professional RN experience (home health experience preferred)
Current BLS or ACLS certification from the American Heart Association
Valid drivers license reliable transportation and current auto insurance

Preferred
Prior experience in home health or community-based nursing
Excellent communication assessment and time management skills
