Our client is seeking a Licensed Practical Nurse to join their Hospice care team in Glen Carbon IL. This position offers the opportunity to provide compassionate high-quality care to patients in their homes while collaborating with a supportive healthcare team. The LPN will assist in patient care plan management ensure accurate documentation and provide exceptional service throughout the assigned area.

Responsibilities include delivering and managing patient care plans supporting patients in Glen Carbon and nearby communities maintaining documentation accuracy managing time efficiently and traveling to and from patient homes as needed.

Schedule: Work schedule and shift details will be confirmed during onboarding.
Travel: Local travel within the assigned area is required.

Required qualifications:

Active Licensed Practical Nurse (LPN) license in the State of Illinois
Associate degree or higher in Nursing
Two or more years of hospice experience preferred
Proficiency with EMR systems such as TruChart
Valid drivers license reliable transportation and auto insurance
