We are looking for a dedicated and compassionate Resident Care Coordinator to join our team on a FT 2pm-10pm shift Tuesdays through Saturdays. This position requires an active LVN or LPN license and plays a key leadership role in supporting the care team. The Resident Care Coordinator reports directly to the Resident Care Director and the role combines leadership and hands-on care responsibilities requiring strong communication skills sound judgment and the ability to foster a collaborative service-oriented environment.
Job Responsibilities:
- Review 24-hour log
- Receipt and record review for new move-in
- EMR Dashboard review and update
- Census review and update
- Review of incidents and regulatory required reporting
- Pharmacy Order updates ineMAR
- Document uploading
- Complete resident assessments and service plans
- Attend and participate in meetings
- Provide supervision and guidance to care associates
- Participate in on-call rotation for after hours or weekend care-related concerns
