Job Duties
Position Summary
Oversee and promote the residents health and wellness through regular visits with each resident resident assessments and ongoing communication with the resident care team.
REQUIRED LEADERSHIP COMPETENCIES
1. Communication: Strong written and verbal skills are demonstrated in reports correspondence and presentations. Ensures staff are kept informed of Medical Center and departmental priorities. Communicates vision statement and goals and demonstrates commitment toward their achievement.
2. Professional Development: Participates in conferences workshops and other professional development activities to maintain licensure and/or remain professionally current with advances in field of expertise.
3. Staff Development: Ensures staff receive mandated in servicing in Safety Quality Assurance Risk Management Customer Service Infection Control and any other in-service that may apply.
4. Problem Solving: Recommendations and decision making reflect strong analytical skills and focus on quality cost containment and impact of change on other departments.
5. Customer Service Management: Incorporates the Medical Centers Customer Service goals in developing and/or revising departmental policies and systems. Continually reviews the service delivery process to exceed customer expectations. Is perceived as a role model by staff peers in customer service leadership.
6. Project Management: Participates in multidisciplinary task forces committees and projects demonstrating team spirit and ability to work with different internal customers. When leading a project team demonstrates the ability to bring together different views and skills toward timely and effective completion of project objectives.
7. Clinical Leadership: Clinical management staff provide clinical supervision. Provide in-service training and maintenance of clinical standards. May provide leadership for hospital-wide clinically oriented committees.
Responsibilities
The LPN works cooperatively with other staff members.
Dispensing medications
Supervising Certified Nursing Assistants Caregivers/Aides
Communicating problems to the Director of Health Care Services
Follow the six rights of medication administration (right resident right drug right dosage right time right route right record/documentation) each and every time assistance with or administration of a medication occurs.
Check each of the six rights of medication administration three different times:
When the medication is taken out of the medication cart or cabinet
Immediately before assisting with/administering the medication.
Immediately after assisting with/administering the medication.
Maintain a neat clean organized medication cart/cabinet and restock the cart/cabinet as needed.
Provide residents with any treatment as part of their individual service plan as designated.
Work in cooperation with all disciplines e.g. the Physician Dietary Pharmacy Social Workers and all other consultants (occupational and speech therapy) communicating resident needs.
Directly supervise the Caregivers.
Participate in Quality Assurance activities regarding the nursing service as directed by the Director of Health Care Services.
Supervise Caregivers to ensure respect of Residents Rights.
Participate in educational in-services provided by the Director of Health Care Services.
Pass all competency tests defined by the organization.
Observe all residents carefully and report any changes or problems to the Director of Health Care Services and document this information in the Communication Log with date time and signature.

Resident Services
Provide quality care and assistance to residents in accordance with the residence philosophy policy and procedure and resident rights.
Assist residents in meeting their physical emotional spiritual and medical needs to achieve and maintain their highest level of wellness.
Promote and protect the residents rights to safe prompt and confidential treatment.
Answer and respond immediately to resident calls.
Respect the residents right to refuse medication/treatment.
Protect the residents safety with medications.
Explain to the residents what medication is being given and why prior to assisting with/administering the medication.
Observe the resident for any adverse reaction to medication and report it to the Director of Health Care Services immediately.
Maintain confidentiality of residents records.
Control the spread of infection by following standard precautions.
Dispose of infectious and potentially infectious waste according to residence policy and procedure and OSHA regulations.
Establish and maintain a good relationship with residents and their families.
Respond to resident emergencies following proper procedures.
Other Duties
Performs other duties as assigned to support the overall objectives of the department and organization.
Position Qualifications
Must have good character.
Excellent communication skills.
Compassionate and able to handle stress.
Ability to sit walk and stand for prolonged or considerable periods.
Able to lift position and transfer patients supplies and equipment.
Considerable reaching stooping bending kneeling and crouching may be necessary.
Interest in working with older adults.
BLS Certification is .
IV Certification is preferred.
Education
High School Diploma or G.E.D. equivalency certificate is .
Must have an unexpired NJ State LPN License from the New Jersey State Board of Nursing and/or a Compact Nursing License giving you access to practice in the state of NJ.
Job Setting/Physical Demands
To fulfill job requirements the employee must be able to perform key physical tasks for extended periods of time.
When supporting lifting or transferring residents proper body mechanics and safety measures as prescribed by the residence must be used.
In a typical eight-hour day the employee may:
Stand/walk up to six hours a day.
Sit up to two hours a day.
Frequently support up to 70 pounds
Occasionally lift/cany up to 15 pounds.
Frequently kneel bend and reach.
Secure proper assistance for transferring and lifting of residents as needed.
