We are seeking a compassionate and skilled Registered Nurse (Home Health) to join our clients growing team in Elgin Illinois. This position involves providing quality nursing care in patients homes supporting their independence and enhancing their health outcomes. The ideal candidate will have strong clinical skills effective communication and a commitment to patient-centered care.

Key Responsibilities
Deliver professional nursing care and conduct comprehensive patient assessments in home settings
Develop individualized care plans in collaboration with physicians and healthcare teams
Educate patients and families on treatment plans medications and post-care instructions
Monitor patient progress and adjust care as needed to ensure optimal outcomes
Maintain accurate and timely documentation according to regulatory standards
Coordinate effectively with the care team to ensure seamless patient care

Schedule and Shift Details
Full-time Monday through Friday
Flexible schedules no long shifts minimal weekend shifts annually
Comprehensive 6-week RN onboarding program
This is a Pay Per Visit position

Travel
Local travel within the Elgin area required (travel reimbursement provided)

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Illinois
Minimum of 1 year professional RN experience (home health experience preferred)
Current BLS and CPR certification through the American Heart Association (assistance available if needed)
Valid drivers license reliable vehicle and current auto insurance

Preferred
Prior experience in home health or community nursing
Strong assessment communication and organizational skills
