Overview
We are seeking a compassionate and skilled Registered Nurse (Home Health) to join our clients expanding team in Toledo Ohio. This position focuses on delivering high-quality patient-centered care in a home setting helping individuals achieve their health goals and maintain independence. The ideal candidate will bring strong clinical expertise excellent communication skills and a commitment to providing exceptional patient care.

Key Responsibilities
Conduct comprehensive patient assessments and provide skilled nursing care in home environments
Develop implement and evaluate individualized care plans in coordination with physicians and other healthcare professionals
Educate patients and families on medical conditions medication management and self-care practices
Monitor patient progress document clinical observations accurately and adjust care plans as needed
Ensure compliance with all regulatory and documentation standards
Collaborate with the interdisciplinary team to promote optimal patient outcomes and continuity of care

Schedule and Shift Details
Monday through Friday 8:00am 5:00pm
One weekend per month (on-call)
Two holidays per year
Flexible scheduling with minimal weekend work
This is a Pay Per Visit position

Travel
Local travel required throughout the Toledo area (travel reimbursement provided)

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Ohio
Minimum of 1 year of professional RN experience (home health preferred)
Current BLS or ACLS certification through the American Heart Association
Valid drivers license reliable transportation and current auto insurance

Preferred
Prior experience in home health or community-based care
Strong organizational communication and assessment skills
