Location: Alameda CA
Pay Range: $42.00 $47.00 per hour (approx. $93000 annually at $45/hour)
Setting: Skilled Nursing Facility (SNF)
Schedule: Full-time Flexible weekday shifts

WellTech Partners is seeking a passionate and dedicated Physical Therapist Assistant (PTA) on behalf of one of our partnered skilled nursing facilities in Alameda CA. This is an exciting opportunity to join a team committed to delivering exceptional rehabilitation services and improving the quality of life for patients in a supportive team-oriented environment.

Position Summary

The Physical Therapist Assistant will work under the supervision of a licensed Physical Therapist to provide high-quality hands-on therapy services that help patients regain mobility independence and confidence. The PTA will assist in implementing treatment plans track progress and support the therapy team in maintaining outstanding patient outcomes.

Key Responsibilities

  • Deliver individualized patient treatment as outlined by the supervising Physical Therapist.

  • Provide clear instructions and education to patients and families regarding exercises mobility techniques and care plans.

  • Communicate effectively with supervising PTs and interdisciplinary team members about patient progress challenges and discharge planning.

  • Document treatments and progress notes accurately and in compliance with facility state and regulatory standards.

  • Assist with scheduling documentation and maintaining accurate patient billing records.

  • Participate actively in patient care conferences interdisciplinary meetings and therapy department initiatives.

  • Maintain a clean organized and safe treatment environment while adhering to universal safety precautions.

  • Support the professional growth of self and others through continuous learning and collaboration.

  • Promote a positive patient-centered culture by demonstrating empathy teamwork and professionalism at all times.

Qualifications

  • Education: Associate of Science Degree from an accredited Physical Therapist Assistant (PTA) program.

  • Licensure/Certification:

    • Current California PTA license (or eligibility for licensure).

    • Current CPR certification.

  • Experience:

    • Prior experience in geriatric rehabilitation preferred.

    • Experience with neurological cardiac or general rehabilitation is an asset.

  • Skills & Abilities:

    • Knowledge of physical therapy treatment techniques therapeutic modalities and assistive devices.

    • Excellent communication and interpersonal skills.

    • Strong documentation and organizational abilities.

    • Compassionate approach with a commitment to patient safety and comfort.

Physical Requirements

  • Ability to lift and transfer patients (50-70 lbs) multiple times per day.

  • Frequent standing walking bending and reaching during treatment sessions.

  • Capability to perform CPR and other emergency procedures if required.
