Responsibilities
Works collaboratively with the Case Management staff and other health care team members to coordinate and execute discharge planning services for patients through the healthcare continuum. Assists in the execution of a plan of care to facilitate a safe discharge in a timely manner and serves as a liaison on community facilities and and Case Management documentation standards as indicated in CM policy and protocols.
Qualifications
Education
One of the following is required
o Bachelors Degree in Social Work or related field
o Completion of an accredited Vocational Nursing School program
Experience
1 year of experience in Case Management with prior assessment and referral experience required
Licenses and Certifications
BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
LVN - Current State of California Licensed Vocational Nurse License for incumbents without a qualifying Bachelors Degree required
