Key Responsibilities

Collaborate with the team to provide management and delivery of patient care plans
Support patients in South Park and surrounding areas
Consistently meet critical charting and documentation deadlines
Self-manage time and scheduling effectively
Utilize tablets mobile phones and EMR software proficiently

Schedule and Shift Details

Monday through Thursday 5:00 p.m. to 8:00 a.m.
May consider Monday through Friday 5:00 p.m. to 1:30 a.m. schedule

Travel

Local travel required within the service area

Qualifications

Required

Associate degree or higher in Nursing
Minimum of 2 years hospice nursing experience
At least 1 year experience with Electronic Medical Records (EMR) including TruChart EHR or other systems
Valid Registered Nurse (RN) license in Pennsylvania
Valid Drivers License reliable transportation and auto insurance
Comfortable with the travel requirements for this position

Preferred

RN license number available upon application
