Overview
We are seeking a dedicated and compassionate Home Health Physical Therapist to join our care team in Western this role you will provide home-based physical therapy services helping patients achieve their highest level of physical function and independence. Key Responsibilities

Deliver home health physical therapy services to patients across Johnstown Kittanning and surrounding areas
Conduct comprehensive assessments and evaluate each patients physical capabilities
Develop and implement individualized care plans
Provide therapeutic interventions to improve mobility strength and overall function
Educate patients and families on exercises care routines and safety
Maintain accurate and timely documentation in the EMR system
Collaborate with interdisciplinary team members to ensure coordinated high-quality care

Schedule and Shift Details

Monday through Friday 8:00 AM to 5:00 PM
Full-time schedule
One weekend per month and two holidays per year

Travel

Local travel to patients homes in Western Pennsylvania

Qualifications

Required

Bachelors or Masters degree in Physical Therapy from an accredited institution
Active Physical Therapist license in the state of Pennsylvania
Minimum of one year of healthcare experience
Proficiency in Electronic Medical Record (EMR) systems
Valid drivers license and reliable transportation for local travel (20% of the day)
Strong time management and organizational skills
Ability to work independently and meet clinical documentation deadlines

Preferred

Prior home health experience
PT license number provided upon application
