Key Responsibilities
Manage and deliver individualized patient care plans
Conduct comprehensive assessments and implement appropriate treatment plans
Collaborate with interdisciplinary care teams to ensure coordinated care
Educate patients and caregivers on exercises and functional mobility techniques
Maintain timely accurate and compliant documentation in EMR systems
Promote patient independence and safe home environments
Schedule and Shift Details
Monday Friday 8:00 AM 5:00 PM (some flexibility available)
Travel
Local travel to patient residences within assigned territory
Qualifications
Required
Bachelor s degree or higher in Physical Therapy from an accredited institution
Valid Physical Therapist license in Michigan (MI)
Minimum 1 year of healthcare experience
Experience using Electronic Medical Record (EMR) systems
Valid driver s license and reliable transportation
Preferred
