Job Summary
Saint Joseph is seeking a Urologist to assist with clinic and call coverage!
Dates of coverage needed (ongoing needs continued into 2026 TBD):
- Friday October 24 at 5a Monday November 3 at 9a
- Monday November 10 at 7a Friday November 14 at 5p
- Monday November 24 at 7a Thursday November 27 at 5p
- Monday December 1 at 12noon Friday December 5 at 5p
- Monday December 8 at 7a Friday December 19 at 5p
- Monday December 22 at 7a Friday December 26 at 5p
- Clinic and Call coverage. Clinic will be 7a-5p daily Night call 5p-7a. Ideally they would like a provider to cover clinic/call during the week however the client will also accept a provider willing to take 24 hour call without clinic on these dates!
- Will start with sporadic dates but are ultimately seeking someone who can offer more consistent coverage for a long term assignment.
- Adults only
- 2-5 ppd
- EMR: EPIC
- General Urology procedures required
- Willing to pay for full travel
Credentialing Timelines:
a. Payer enrollment: 90-120 days from receipt of Demographics IN license IN Controlled Substance Registration DEA w/IN practice address and signed enrollment forms.
b. Hospital Privileges: 45-60 days from receipt of complete CVO/CPI Portal Application.

Requirement description :
- BC or BE within 5 years of residency - REQUIRED
- IN license at the time of submission or IMLC - REQUIRED
- CDS Certification and DEA Registration - REQUIRED
- ACLS - REQUIRED
- Robotics trained - HIGHLY PREFERRED - Please note whether the provider has this training at the time of name clear
- 25K Conversion Rate - REQUIRED
- Please list availability in submission at the time of name clear! - REQUIRED
CERTIFICATION REQUIREMENTS :
- ACLS
- Board Certified
STATE LICENSE REQUIREMENTS :
- Indiana
ADDITIONAL LICENSE REQUIREMENTS :
Weekend Requirements : Occasional weekends needed!
On Call Requirements : Night call 5p-7a
