Overview:
As the Program Supervisor of Integrated Health (IH) you would support a team of direct service providers working across Mendocino County. You would supervise care managers providing ECM to people experiencing homelessness children families people with severe mental illness and people with substance use challenges. ECM provides support to highly vulnerable participants through intensive coordination of health and health-related services. ECM providers coordinate care and services among the physical behavioral dental developmental and social services delivery systems making it easier for participants to get the right care at the right time. These services improve access to care for people with complex needs and who are facing difficult life and health circumstances. This is a full-time position working in team group and one-on-one settings with Care Managers Peer Support Counselors behavioral health providers foster family services support service providers community partners and clients. You would work collaboratively with the Program Manager and Director to support the service team to deliver trauma-informed compassionate services. You would also work with the teams at other agency projects to serve children youth and adults. You would be responsible for scheduling training and coaching program staff to build a team that ensures quality service and care. You would monitor the provision of client services and documentation providing direct client services as needed. You would work collaboratively with other service providers and agencies to improve access to housing and other resources necessary to improve quality of life. As a member of the leadership team for our homeless and housing programs in Mendocino County you would be on a rotating schedule to provide 24 hour on call support for the Building Bridges emergency shelter and the permanent housing projects RCS operates. As IH Supervisor you would aid with the expansion and development of ECM across the county. Seeking candidates with experience with MediCal managed care plans health care systems homeless services continuum of care and/or children welfare systems. Ideal candidates are creative leaders who think outside the box and enjoy developing new systems and processes.
Key Responsibilities:
- The Enhanced Care Management (ECM) Supervisor is the direct supervisor for 8-15 staff. These staff are stationed in RCS Building Bridges homeless resource center Stepping Stones behavioral health services childrens behavioral health services and foster family departments. This supervisor has a high level of coordination with those departments leadership to support the integration of ECM.
- Responsible for the overall direction coordination and implementation of ECM in collaboration with the Program Manager and in accordance with the applicable policies and regulations.
- Provide 24 hour on-call support primarily over the phone but occasionally in person to staff at the emergency shelter and permanent housing facilities in Ukiah and Redwood Valley. This is on a rotating schedule with other program leadership.
- Support ECM providers with building and maintaining their caseloads. Monitor services delivered to ensure all clients are being seen regularly and that inactive clients are moved off caseloads in a timely manner. Manage the assignment of new clients as room on caseloads allows.
- Ensure the appropriate documentation and reporting is completed in a timely fashion when there is an injury serious incident safety issue maintenance need or other unusual circumstance.
- Under the direction of the Program Manager and in consultation with Human Resources participate in the recruitment training and supervision of all program staff. Creation of training and onboarding materials in coordination with the Manager and Training Department.
- Create and uphold a creative team-centric compassionate judgment free and collaborative culture and approach to guest services and job performance; model this behavior and coach staff to develop a solution-oriented environment with the intention to engage and retain guests.
- Support team with creating and following through with individualized care/housing plans for each client.
- Maintains real-time client lists and waiting lists. Ensures that each client accepted is assigned a lead Care Manager.
- Intervenes and de-escalates situations that would lead to dangerous behavior. Recommends or initiates other non-physical measures to redirect behavior. Exits individuals from the facility according to the policies and communicates to staff about the details of the persons exit and terms for re-entry.
- Ensure that all required documentation is completed and entered in the appropriate places in a timely manner. Ensures information is accurately entered in the appropriate databases as required.
- Ensure the program is compliant with funding objectives and expected outcomes. Conducts implementation of program and agency Policies & Procedures. Assists in developing and analyzing outcome measures.
- Serve as an information resource by regularly reviewing research on best practices assembling data and performing special projects; prepare and deliver presentations reports correspondence and documents.
- Communicate and collaborate with partnering service providersregarding needs progress and care of guests as allowed by regulations permitting the sharing of information between providersto benefit their clients accessing shelter housing and in community meetings especially ones focusing on case conferencing clients who utilize our services.
- Develop and maintain cooperative and professional relationships with public and private social service agencies to enhance programming for participants.
Education:
Strongly desired College degree in social work or related field strongly desired.
Experience:
Minimum of one (1) year of supervisory experience. Minimum of three (3) years of case management experience in a nonprofit social services setting.
Strongly desired lived experience with child welfare system homelessness mental illness substance use challenges etc.
Strongly desired experience working with people experiencing homelessness mental illness substance use child welfare system etc. Strongly desired Experience and/or training in harm reduction Housing First trauma-informed low barrier service environments.
Strongly desired Experience with data management reporting and data analysis systems. Must have basic knowledge of Microsoft Suite (Excel PowerPoint Outlook etc.).
Conditions of Employment:
- Must possess a valid CA Drivers License current auto insurance and a clean DMV printout (no more than 3 points).
- Must pass a clear background check with DOJ & FBI.
- CPR and First Aid Certified.
- TB Exam and Physical at the time of employment.

- Driving is a required function of this position both company and personal vehicles. Must use a personal vehicle that meets all agency requirements including maintaining liability insurance that meets or exceeds state requirements.
- Employees are responsible for ensuring the timely renewal of their vehicle insurance policies and providing updated documentation to the agency to remain in good standing and compliant with position requirements.
On-Call Rotation Responsibilities:
- Participate in the programs on-call rotation.
- Commit to a minimum of one weekend per month.
- Sign and adhere to the on-call agreement.
- Develop and maintain the on-call schedule for the program.

- Training: Preferred Training in harm reduction Housing First trauma-informed low barrier service models.
