What Youll Do
-Coordinate home and community-based services for individuals with disabilities and seniors
-Build trusting relationships with clients and families
-Train schedule and support direct care staff
-Conduct client intakes visits and assessments
-Collaborate with agencies and partners to deliver high-quality care
-Maintain accurate client records and participate in on-call rotation

What You Bring

-Bilingual (English/Spanish) required
-1 year of experience in direct care case management or related work
-A calm resourceful approach to challenges
-Reliable transportation for travel across Southern AZ
-Ability to obtain CPR/FA certification fingerprint clearance DES background check and TB card
