Job Summary
Hospice Nurse Practitioner
Salary: $115k-150k/ per year BOE
Schedule: Full-time Monday- Friday 8:00 AM- 4:00 PMOC
Location: Toledo Ohio and surrounding area
Requirements of the Nurse Practitioner role:
Must have completed an accredited Nurse Practitioner program
Minimum of 2 years of nursing experience in-home hospice or skilled home care.
Current nursing licensure in the state of practice
Maintains controlled substances registration with state and federal authorities
Not excluded from the medicare program
About the Nurse Practitioner role:
As a Nurse practitioner you will be responsible for providing hospice services including but not limited to complementing attending physician care acting as a medical resource in IDG performing face-to-face visits as needed evaluating hospice eligibility as assigned in consultation with the hospice interdisciplinary team and assuring that the patient receives appropriate measures to control uncomfortable symptoms.
Collaborating with the IDG to ensure that the needs of patients are met
Ensuring the patient meets medical criteria for hospice admission based upon available diagnostic and prognostic indicators.
Review clinical information coordinate and oversee services to patients either directly or through arrangements as appropriate in the absence of the patients attending physician.
Attend IDG conferences as needed
Participates in the plan of care development and managing oversight of medications and treatment.
Review and develop protocols for treatment and propose the most current options for interventions.
Support on-call needs as scheduled to ensure continuity of patient care.
