Job Summary:
The nurse navigator functions on a multidisciplinary team as an advocate educator and communicator for the oncology patient and their families.
Responsibilities
Educates patients and families about disease process treatment options and potential side effects.
Coordinate patients through the continuum of care in collaboration with the multidisciplinary team.
Serve as the liaison between disciplines and any supportive/community services.
Coordinate weekly multidisciplinary conferences and database tracking analysis for treatment outcome purposes.
Compile data tracks outcomes and make recommendations for process improvement.
Provide referrals/support for follow-up care for patients.
Travel to multiple locations with the ULH system as needed.
Required Qualifications
Graduation from an accredited/approved school of nursing.
Associate Nursing Degree required BSN required within 2 years of hire.
Current RN license in Kentucky.
2 years of oncology experience at the bedside.
Excellent communication skills and the ability to interact with nurses physicians and skilled healthcare professionals.
Basic Life Support (BLS) certification from the American Heart Association.
Ability to work autonomously without direct supervision.
Preferred Qualifications
Oncology certified nurse (OCN) recommended or to be obtained within 2 years of employment.
Academy of Oncology Nurse and Patient Navigator certification recommended or to be obtained within 2 years of employment.
Previous Cerner experience.
