Rocky Mountain Care is hiring for a Transitional Care Specialist to join our Payson UT team!
Required Qualifications:
- Strong interpersonal and communication skills
- Understanding of the emotional and logistical complexities of transitions in care
- Detail-oriented organized and empathetic approach to patient interactions
- Experience in healthcare or care coordination preferred but not required
Major Duties and responsibilities:
- Greet new admissions upon arrival and provide a clear overview of what to expect during their stay
- Serve as the welcoming face of the facility to ensure patients and families feel informed and supported
- Educate patients and families on the next level of care including home health hospice or physician follow-up services
- Conduct regular patient rounding to identify needs monitor satisfaction and resolve concerns promptly
- Ensure timely and seamless transitions across all services within the Rocky Mountain Care continuum
- Provide facility tours share the mission and vision of Rocky Mountain Care and promote services during interactions with prospective residents and families
- Collaborate with patient care coordinator interdisciplinary team members and referral sources to support smooth transitions and patient outcomes
Additional Responsibilities:
- Promote a five-star patient experience by aligning with the Rockys values and mission
- Act as a resource for residents and families during their stay answering questions and offering guidance
- Support documentation reporting and communication processes to enhance care coordination and service excellence
- Participate in internal performance improvement efforts and facility initiatives related to care transitions
- Demonstrate professionalism and compassion in every interaction with patients families and colleagues Reports barriers to smooth transitions within the continuum to Director of Transitional Care

- Demonstrates an understanding of arithmetic.
- Must have as a minimum one (1) year of experience with demonstrated success in a position in the facility or in another related health care facility.
- Knowledge of insurance coverage and authorization process pertaining to long-term care is helpful.
- Knowledge and experience in working with multiple post-acute continuum of care services.
- Demonstrates strong communication and people skills with the ability to market and grow facility census as well as RMC continuum care transitions.
