Job Summary:

Navitas Healthcare LLC is seeking Licensed Clinical Social Workerfor an exciting job in Hyannis MA.

Requirements:

Active Massachusetts LCSW License (Required)
Crisis Prevention Intervention (CPI) Certification
Basic Life Support (BLS) Certification
Excellent communication and clinical documentation skills
Experience in behavioral health psychiatric or acute care settings preferred
Strong interpersonal organizational and crisis management skills

Responsibilities:

Conduct comprehensive clinical assessments for patients with behavioral health needs.
Develop and communicate individualized treatment and discharge plans.
Provide therapy and counseling services including individual group and family sessions.
Collaborate with psychiatrists nurses and other healthcare professionals to ensure quality care.
Coordinate with community agencies and services to support patients post-discharge needs.
Assist patients and families with planning resources and placement for continued care.
Provide crisis intervention and prevention support as required.
Maintain compliance with state and federal regulations and documentation standards.
Participate in care coordination meetings and community outreach programs.
Deliver compassionate patient-centered care and maintain professional relationships.

Education:

Masters Degree in Social Work from an accredited institution (required)
