Program Support Housing for Families
Location Middletown (Roscommon Office)
Salary - $27.00/hour
Schedule Full time 40 hours 1st shift Monday-Friday
Program Summary:
A child welfare program providing intensive case management vocational supports and temporary housing subsidies to families that are homeless or at risk of homelessness in order to support the achievement of family reunification and/or prevent the removal of children from their families.
Position Summary:
The Clinical Assessment Specialist conducts interviewswith high and low risk families referred from the Department of Children and Families to determine appropriate levels of care within the SHF program. Responsibilities include but are not limited to scheduling and meeting with clients in their homes to perform detailed clinical assessments collaboration with treatment providers documentation of information collected and making appropriate referrals when necessary.
This position is non-exempt and works under the supervision of Program Directorship. This position is based out of the Middletown office but services clients throughout the state of Connecticut. Agency vehicles are provided for travel.
Working within our Family & Child Services the Clinical Assessment Specialist works with the Supportive Housing staff to provide services to stabilize and reunite families experiencing complex issues such as substance use behavioral health matters and homelessness.
