Job Summary
Provide a wide range of psychosocial evaluation/assessment diagnostic counseling therapy crisis intervention group facilitation and/or case management services in an integrated multi-disciplinary primary care setting located in Central MA. Work collaboratively with other mental health and medical providers and trainees engaged in related therapeutic/patient care activities.
Primary Responsibilities
- Provides individual group and family therapy to clients as appropriate to the position; prepares treatment plans and case management including discharge planning; provides therapeutic crisis stabilization services as required.
- Maintains relationships with referral partners; collaborate with outside treatment providers agencies in relation to patient/client records rights and responsibilities while engaged in day treatment.
Requirements
- Masters degree in Counseling or Social Work.
- Licensed Clinical Social Worker (LICSW) Licensed Mental Health Counselor (LMHC) or equivalent.
