Position Summary: The RN Wound Care conducts health assessments with a focus on wound ostomy and continence-related care providing expert patient-centered treatment plans and education. This role collaborates with the interdisciplinary health care team to deliver high-quality care advances standards in wound management and mentors staff and patients for best practices and prevention.
Key Responsibilities Perform focused health interviews and physical assessments on patients with wounds ostomies continence concerns and fistulas designing individualized care plans. Recommend and administer medications topical treatments and wound therapies including negative pressure and specialty equipment asset management. Develop revise and evaluate wound care standards and assist in product selection (supplies equipment technology). Routinely assess and document patient progress collaborate with nursing leadership to promote evidence-based best practices and participate in QI studies and hospital-wide initiatives. Evaluate clinical staff delivery and quality of wound care on the unit provide referral guidance and escalate care or ethical concerns as needed. Provide education and counseling for patients families and staff with special emphasis on culturally competent prevention and complication reduction. Lead and contribute to staff orientation and in-service training programs including regulatory and continuing education requirements. Foster collaboration and communication across disciplines to promote patient outcomes and address barriers to care. Identify community resources for wound care management and support successful patient transitions.
Required Skills & Experience Minimum 2 years of wound and colostomy nursing experience in an acute care setting. Proficiency in clinical wound assessment documentation and evidence-based interventions.
Education: Bachelors of science in nursing preferred. Associate Degree (ASN ADN) from an accredited Nursing Program required.
Certifications & Licensure Active New York State RN License with primary source verification (NYSED). American Heart Association BLS. One of the following current certifications: Certified Wound Ostomy Continence Nurse (CWOCN) WOCNCB Certified Wound Ostomy Nurse (CWON) WOCNCB Certified Wound Care Nurse (CWCN) WOCNCB Certified Wound Specialist (CWS) ABWM Wound Care Certified Certification (WCC) NAWCO
