Job Summary
We are seeking a highly motivated and experienced Registered Nurse to join our team as a Patient Care Manager in the Home Health sector based in Roswell. This role is key in overseeing and ensuring the delivery of high-quality compassionate care to patients within their homes.
Key Responsibilities:
- Lead and supervise a dedicated team of healthcare professionals to deliver exceptional patient care.
- Coordinate patient care services and develop individualized care plans.
- Conduct regular patient assessments and monitor the effectiveness of care plans.
- Ensure compliance with all healthcare regulations and standards.
- Collaborate with multidisciplinary teams to optimize patient outcomes.
- Provide education and support to patients and their families regarding health conditions and care plans.
- Maintain accurate and timely documentation of patient care and progress.
Qualifications:
- Current Registered Nurse (RN) license in the state of Georgia.
- BSN degree preferred or equivalent clinical experience.
- Home Health leadership experience required.
- Experience in home health care or a related field.
- Previous experience in home care setting with two years management or supervisory experience preferred.
- Strong communication and interpersonal abilities.
- Excellent organizational and problem-solving skills.
- Proficiency in using electronic medical records and other healthcare technologies.
