8-Hour Evening Shift: 3:00 PM 11:30 PM 40 hours/week guaranteed
12-Hour Evening Shift: 11:00 AM 11:00 PM 36 hours/week guaranteed

Job Description:

We are seeking experienced and dedicated Licensed Practical Nurses (LPNs) / Licensed Vocational Nurses (LVNs) for a 13-week travel contract in a Long-Term Care facility located in Watertown NY. The ideal candidate will be passionate about geriatric care and have a strong background in either LTC or SNF environments. This is a great opportunity to work in a supportive team while gaining valuable experience in travel nursing.

Responsibilities:

Provide direct nursing care to residents in accordance with their individualized care plans.
Administer medications and treatments as prescribed by the healthcare provider.
Monitor residents health conditions and report any changes to the supervising RN or physician.
Assist with ADLs (Activities of Daily Living) and ensure residents comfort and safety.
Document all care provided using the facility s EMR system (PointClickCare).
Collaborate with other healthcare professionals to ensure quality patient care.
Maintain a clean and safe environment for residents.

Experience Required:

1-3 years experience required.
SNF (Skilled Nursing Facility) experience; LTC-only experience acceptable.
EMR Experience: Prior use of PointClickCare EMR system preferred.

License & Certification:

Active LPN License in New York (NY) Required
BLS Certification (American Heart Association) Required
