Full-Time M-F 9am-5pm

Our client a mission-driven nonprofit health organization providing comprehensive care to underserved New Yorkers. Were seeking a skilled and compassionate Licensed Clinical Social Worker (LCSW) to join our East Harlem team.
What Youll Do
- Provide clinical social work services including assessments counseling and psychotherapy in an inpatient care setting.
- Collaborate with medical and behavioral health providers to develop and implement patient care plans.
- Coordinate referrals entitlement support and community services to address patient needs.
- Support HIV prevention efforts through adherence support data collection and follow-up.
- Document services in our EHR and ensure compliance with HIPAA and organizational policies.
- Assist in crisis situations involving child abuse domestic violence or elder abuse.
- Contribute to quality improvement initiatives and care coordination meetings.
What Our Client is Looking For
- LCSW licensed in NYS with 3 years post-MSW experience including 1 year in an outpatient/ambulatory setting.
- Experience with chemical dependency MAT and HIV services preferred.
- Strong interpersonal organizational and communication skills.
- Culturally competent and comfortable working with diverse communities.
- Bilingual (English/Spanish) strongly preferred.
- Proficient in Microsoft Word and EHR systems.
