Job Summary

What You Get to Do: 

  • Coordinating a variety of services including but not limited to; individual finances community integration program implementation family involvement and education/vocational programs
  • Program planning assessments and implementation
  • Communicating with the Nurse Consumer Benefits Coordinator Health Service Coordinator and maintenance staff
  • Assist in direct training follow up and coaching with program staff
  • Build relationships with persons served and teammates.
  • Provide hands-on assistance encouragement mentoring and guidance.
  • Ensure the comfort safety and personal growth of the individuals we serve.
  • Assist with daily living tasks such as moving around getting in and out of bed dressing bathing personal hygiene and more.
  • Light housekeeping tasks and meal preparation.
  • Attend special community occasions and fun activities like outdoor walks games and social events.
  • Transport persons served to appointments or other activities outside the home.
  • Empower people with disabilities to live life to the fullest.
  • Document progress milestones and action steps.

Qualifications :

What Makes You A Great Fit:

  • 1 3 years of experience in residential and/or support services for individuals with developmental disabilities
  • Leadership skills observant and detail oriented
  • Person-centered patient and kind
  • Dependable adaptable flexible
  • Positive role-model for others and able to work on a team
  • Committed to creating a respectful and collaborative environment
  • Computer skills for documentation
  • 18 years or older high school diploma/GED and basic computer skills
  • Successful clearance of a criminal background check for licensing requirement
  • Valid Drivers License personal vehicle and liability insurance
