POSITION SUMMARY
The Host Home Professional Manager will support the CHS mission by working directly with Host Home Providers in their assigned region to ensure compliance with organizational policies and procedures Medicaid Waiver guidelines state contracts and other applicable governance toward the result of excellent care for the individuals they support.

ESSENTIAL DUTIES AND RESPONSIBILITIES
As the Host Home Professional Manager you will:

  • Support the successful care of individuals by providing input and perspective into outcomes and goals for individuals served in Host Home services; protecting individuals rights ensuring they are not limited and initiating due process when required; observing for abuse neglect or misuse of individuals funds during regular visits to Host Homes; conducting quality assurance checks and environmental site reviews; and traveling throughout the assigned region in company-owned fleet vehicles when available to conduct business with Host Home providers relief providers and coordinating agencies;
  • Increase the amount of accessible care by building relationships with coordinating agencies and other stakeholders; recruiting new Host Homes and actively pursuing matches of individuals and services within the assigned region; and representing CHS in communities to promote the Host Home model
  • Create a culture of communication by attending and conducting meetings in-person and virtually; communicating regularly with the management team Host Home providers and all relevant stakeholders encouraging cooperation collaboration and minimizing conflict; working closely with Service Coordinators and attending all necessary meetings to establish and maintain accurate and effective individual Support Plans (ISPs); and coordinating with other agency professionals to open communication channels and effectively and collaboratively progress excellence in Host Home services
  • Maintain integrity of records by creating writing editing and reviewing reports and forms; completing checklists; monitoring tracking; recording safety checks; documenting visits; reviewing and recording MARs physicians orders and count sheets for accuracy and compliance; providing governance advice training and support to Host Home providers on implementation strategies documentation medication administration record-keeping individual care and compliance including the monitoring and reporting of progress and outcomes
  • Project a positive image of the Center for Human Services by maintaining a professional appearance and attitude internally and externally; being prepared to capitalize on unexpected opportunities to promote the organization and our services as well as the Host Home model; and embracing inclusion diversity equity and accessibility and promoting the same.

QUALIFICATIONS
REQUIRED EDUCATION and EXPERIENCE

  • Bachelors degree (BA or BS) from an accredited university required (Direct experience or a combination of education and experience may be considered in lieu of education.)
  • Managerial and supervisory experience in a professional environment required
  • Experience working in the field of intellectual or developmental disabilities required
  • Experience working with non-profit organizations preferred

REQUIRED MISCELLANEOUS QUALIFICATIONS

  • Valid drivers license
  • Reliable licensed and insured vehicle
  • Reliable form of communication
  • Ability to complete all mandatory (paid) trainings including First Aid CPR and Level 1 Med Aide

PHYSICAL REQUIREMENTS

  • Lift carry squat bend move push or pull sit or stand for long intermittent periods
  • Type on a keyboard and vision sufficient to view computer screens for extended periods
  • Local driving and intrastate travel consisting of 50% or greater of work time with overnight stays possible
  • Ability and strength to position/lift/transfer an adult human being (greater than 50 lbs.) from varied positions including standing to laying on floor and the ability to continuously push and pull 100-125 lbs. of pressure over an extended period as per the American Heart Association requirements for administering CPR.
  • Continuous ability to kneel on hard or other surfaces for extended periods as per requirements of performing CPR

SALARY: Exempt

SHIFTS: Full-Time (40 hrs. per week). This position has on-call responsibilities.
