SALARY INFORMATION: $66351.55 Annually / JCCP Code HCP 191 / Pay Grade L

Job Responsibilities
This position evaluates the health conditions of People Living with HIV/AIDS (PLWHA) appraising their needs and recording the history of their encounter in a detailed fashion to ensure a comprehensive account of the patients well-being and visit. This position requires the ability to assess plan for and provide basic nursing skills in addition to perform CPR and for some sites AED. Furthermore this position necessitates an extensive knowledge for procedures including the development and implementation and revision of plan of care; nursing policies procedures protocols treatments standards of care and Nurse Practice Act; immunization regulation and protocols; pharmacology; and procedures for medication administration e.g. oral administration sublingual and IV Medications.
Essential Duties and Responsibilities:
Tailor care provision and interventions based on the needs of People Living with HIV/AIDS (PLWHA) and the resources available.
Provides treatments administers medications/vaccines and provides preventive and maintenance care to PLWHA and families consistent with nurse protocols.
Screens and refers PLWHA to other internal and/or external providers as appropriate.
Participates in the planning organizing and evaluation of program areas to ensure the goals and objective(s) of the program are met.
Prepare indicated clinic reports as assigned
Applies principles of population health to practice.
Conducts comprehensive assessments of PLWHAs health care needs.
Develops comprehensive plans of care for PLWHA families and populations.
Modifies PLWHA treatment plans as indicated by patients responses and conditions.
Participates in PLWHA intake and orientation for program services
Provides health education instruction and/or classes to PLWHAs families for the promotion and maintenance of health.
Participates in individualized clinical IT and administrative trainings as assigned
Participates in drills and exercises on pandemic influenza mass sheltering mass dispensing and other areas as indicated.
Performs job responsibilities with minimal supervision.
May serve as lead for various clinic processes as assigned
Adheres to all DKPH policies and procedures.
Perform other duties as assigned.
Minimum Qualifications
Bachelors degree in nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification
OR
Associates degree in nursing from an accredited college or university AND One year of nursing experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification.
Preferred Qualifications:Prior experience working in Public Health and with People Living with HIV/AIDS (PLWHA). Knowledge of HIV Care Continuum and experience providing care to Ryan White patients. Knowledge and usage of computers and Windows Software related programs.
