The Registered Nurse Case Manager provides skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. This role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. Responsibilities:
- Provide skilled nursing care to patients.
- Perform patient assessments and develop treatment plans.
- Administer medications and treatments as prescribed.
- Collaborate with the interdisciplinary team to ensure comprehensive patient care.
- Communicate effectively with patients providers and other team members.
- Manage patient cases ensuring appropriate and timely care.
Required Skills & Certifications:
- Active Registered Nurse license in the state of working.
- Bachelors degree in Nursing or related field.
- Clinical Assessment Medication Administration and Treatment Planning skills.
- Minimum of two years of hospital home health or hospice care experience.
- Interdisciplinary Team Collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Drivers license car insurance and automobile for field work.
Preferred Skills & Certifications:
- Specialities in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room.
Special Considerations:
- Floating expectations at Laguna Facility.
- All requests for time off (RTO) must be submitted upfront.
