Employee Role: Private Duty Nurse (PDN)-LPN
The Private Duty Nurse (PDN)-LPN is a member of the client care team that provides services directly or acts as a placement agency to deliver skilled nursing services to clients in their personal residences. The PDN LPN works under the supervision of the Clinical Manager.
Reporting To: Clinical Manager
Wage/Hour Classification: Non-exempt
Essential Job Responsibilities:
- Evaluates and treats clients with appropriate level of skill according to clients needs under the supervision of an RN Clinical Manager.
- Assist with providing continuity of care and emotional needs of client and family.
- Resourceful in meeting clients psychosocial needs.
- Verbal demonstration of client rights and responsibilities.
- Make decisions based on sound nursing judgements.
- Recognizes and copes with emergencies appropriately.
- Ensures proper management of client services.
- Demonstrates competent use of equipment.
- Appropriately communicates information relative to client care in accordance with agency policies.
- Observes the clients general physical emotional and mental status and reports all changes in the clients conditions family situation incident and occurrences during shift immediately to agency RN/CM or Customer Care Coordinator.
- Ensure all forms of communication with clients families and health plan providers are documented timely to promote continuity of care across all interdepartmental functions.
- Build relationships with clients to encourage compliance with care plans and client safety.
- Maintains accurate clinical and shift work documentations to update system in accordance with guidelines set forth by the agency in a comprehensive and timely manner.
- Manage client/contract satisfaction with the highest level of customer service standards communication and documentation.
- Adheres to infection control policies and exposure control plan as mandated.
- Monitor and maintain clinical report including Infection and Occurrence/Complaint Reports.
- Adheres to and promotes safety and injury prevention measures in client and workplace settings.
- Maintain and improve skills through attendance with staff development programs and in-service education requirements.
- Participate in community outreach activities that promote the goals and objectives of the agency.
- Maintain Confidentiality in all professional areas and consider the needs of clients and their families other employees physicians visitors and all members of the community in an informative courteous and compassionate manner.
- Ensure all HIPAA regulations are adhered to.
- Follow Agency Policies and Procedures
- All other duties as assigned.
ADA Requirements:
- Routinely required to sit stand and walk on variable surfaces including occasional climbing of ramps or stairs or the use of foot controls.
- Routinely required to use hands for various tasks including hand controls gripping and handling of objects.
- At times will be required to reach beyond mid-line of the body reach overhead or at chest level.
- There are specific vision abilities required to perform this job which include close distance and peripheral vision depth perception and ability to adjust focus.
- All employees will be required to use universal safety precautions when performing their assigned tasks.
- Employees must have the ability to lift push pull and carry a minimum of 25 pounds.
- Employees must have the ability to stoop and bend up to 2.5 hours per 8-hour shift.
Requirements
Qualifications:
- Education: Graduate of an accredited school of nursing.
- Licensure / Certification: Licensed practical nurse (LPN) who is licensed by the North Carolina Board of Nursing (NCBON). Must have a current unencumbered license as an LPN or an Advanced Practice Nurse. Must be in good standing in the applicable state of practice. Must maintain all state and federal compliance requirements for licensure and employment.
- Travel: Travel is required for business purposes. Employees must have a valid drivers license issued by the state in which they work and a satisfactory driving record.
- Work Experience: Minimum of one year of home care experience. One additional year of nursing experience preferably in a hospital is required for conducting applicable training classes.
- Other Hours of Work: Varies on a case-by-case basis. Full time is at least 30 hours a week from Saturday-Friday.
- Skills and Additional Requirements:
- A specific understanding of geographical/cultural requirements of branch office is preferred.
- Must be customer service oriented and prepared to liaise with patients their families and other care providers.
- Ability to use computerized systems.
- Satisfactory verbal and written communication skills.
- Ability to work well under pressure.
- Bi-lingual preferred
