Our fast-growing non-medical home care company is looking for a highly organized energetic Care Coordinator to join our team.
Summary:
As a Care Coordinator you play a key role in securing new clients and managing a caseload to ensure exceptional customer service and quality of care. If you are passionate about serving seniors and thrive in a dynamic role this is the job for you. A background in elderly care is essential and an active CNA license is required.
You will work closely with clients and their family members to complete admission assessments develop comprehensive care plans and conduct follow-ups to maintain satisfaction and quality addition you will support scheduling and external marketing efforts to help expand our client base. Our clients and caregivers will depend on your knowledge skills and attention to detail to ensure they are comfortable safe and fully supported.
Work Hours & Location:
This is a full-time exempt position based in our Lombard office with travel throughout DuPage Will and Cook Counties approximately 30% of the time. Mileage will be reimbursed at the federal rate. Regular business hours are Monday through Friday 8 AM 5 PM but the Care Coordinator must also be available for on-call duties every other week with occasional evenings and weekends required.
Job Functions & Responsibilities:
- Assess new prospects prior to starting services.
- Perform follow-up visits to client homes for supervisory and regulatory purposes.
- Create care plans for all new clients.
- Maintain communication with clients and their families regarding care status condition changes and placement opportunities.
- Collaborate with recruiter and scheduler to ensure smooth starts of service and quality care.
- Participate in community outreach and marketing activities to promote Always Best Care services.
- Provide on-call support during assigned weeks addressing urgent client or caregiver needs.
- Ensure compliance with state regulations and company policies through accurate documentation and timely reporting.
Skills & Qualifications:
- Active Certified Nursing Assistant (CNA) license required.
- 2 4 years of customer service experience required.
- Highly organized and self-motivated.
- Ability to manage an active caseload effectively.
- Strong multitasking and communication skills.
- Prior experience in home care coordination or a related field preferred.
- Familiarity with WellSky software is a plus.
- Strong problem-solving skills with the ability to remain calm under pressure.
- Reliable transportation valid drivers license and proof of insurance.
