Licensed Practical Nurse Charter House LPN

Charter Houses mission is to provide the best care through practice education. We are looking for a passionate and dedicated licensed practical nurse (LPN) to join our team. Responsibilities of the LPN role include:

• Providing direct care duties under the delegation of a registered nurse or nurse manager.
• Monitoring the nursing care provided by professional and ancillary nursing personnel.
• Assisting the registered nurse with data collection in collaboration with the resident family and/r significant others and the interdisciplinary health care team to meet the care educational and discharge needs.
• Adhering to Standards of Clinical Nursing Practice and Nurse Practice Act.

To be considered for the LPN position you must meet the following requirements:

• Graduate of a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program (e.g. some states allow RN students to take the LPN board exam after completing one to two years in the RN program).

*If graduation did not occur within the last two years one year of LPN experience working in an applicable setting is required.

Additional Qualifications

• One year of work experience in Long Term Care or Home Health or two years of acute care nursing is preferred.
• Must be able to work in collaboration with others.
• Requires good communication and human relation skills to interact with Health Service and Charter House staff.
• Strong problem-solving skills are necessary with the ability to collect compare sort and prioritize information to be used in the decision-making process for groups of resident and employees.
• Must be able to adapt to unpredictable situations and stress.
• Works flexible hours which may include days evenings nights holidays and weekends.

Licensure/Certifications

MN Licensed Practical Nurse license and Basic Cardiac Life Support for Health Care Providers Certification (must come from one of the following providers: American Heart Association Red Cross or Military Training Network).
