You will be updated with latest job alerts via emailTitle: Patient Navigator - NHMP Danbury Neurosurgery - Danbury CT. Full Time/40 hours per week 8 hour shifts 8:30am - 5:00pm Monday - Friday.
Summary: The Patient Navigator improves the patient experience and care coordination for individuals receiving neurology neurosurgery and PM&R services within the Neuroscience Institute. This individual serves as a liaison between patients families clinical providers and administrative teams ensuring seamless transitions of care optimizing patient engagement and improving access and outcomes across the continuum of care. This role will guide patients to the appropriate subspecialty care centers within the Neuroscience Institute and support referral management as well as the integration of digital health tools to enhance access and communication.
Responsibilities:
1. Serves as a central point of contact for neurology neurosurgery and PM&R patients ensuring timely scheduling follow-up and coordination of care.
2. Assists with navigation into the appropriate clinical care centers and subspecialty services within the Neuroscience Institute.Guide patients and families through the healthcare system post-operative or post-discharge plans in an accessible compassionate manner.
3. Assists with managing patient referrals to both internal and external providers ensuring appropriate follow-up and closure.
4. Addresses barriers to care (transportation insurance communication) and proactively escalate complex cases requiring multidisciplinary input.
5. Collaborates with neurologists neurosurgeons physiatrists nurse practitioners physician assistants and care teams to support pre-operative and post-operative workflows.
6. Assists in tracking clinical milestones such as diagnostic imaging surgical clearance rehabilitation and specialty follow-ups. - Participate in multidisciplinary case reviews as applicable.
7. Supports the adoption and use of digital health tools (e.g. patient portals telehealth platforms remote monitoring devices) to improve communication access and follow-up.
10. Collects tracks and reports key patient metrics (e.g. appointment compliance readmission risk referral completion rates).
11. Identifies trends or gaps in care and contribute to quality improvement initiatives. Proficiently utilize the electronic medical record (EMR) system to document retrieve and analyze patient data across care settings.
12. Maintains accurate and timely documentation to support coordination and communication among the care team. Help implement and maintain patient education materials and communication pathways. Maintain close communication with care teams outpatient coordinators inpatient case managers and social workers.
13. Attends regular Neuroscience Institute team meetings and contribute to program development.
14. Performs other duties as required.
Other Information:
Required: Bachelors degree in Nursing Allied Health or a related clinical field (RN OT PT or similar). Minimum 3 years of experience in clinical care care coordination or patient navigation. Strong interpersonal skills with a passion for patient-centered care. Proficiency in electronic medical Records (EMRs) and data management tools. Demonstrated experience in collecting and analyzing patient care metrics.
Preferred: Experience in neurology neurosurgery or neuroscience care settings. Familiarity with digital health platforms and virtual care workflows. Knowledge of value-based care population health or case management. Bilingual preferred but not required.
Salary Range: $40.43 - $75.10 hourly (This is an exempt position. Salary will be commensurate with experience).
