Description
DEPARTMENT: Adult Day Care
POSITION: Adult Day Care Recreation Leader
SUPERVISOR: Director of Adult Day Care
HOURLY RATE: $17.25 -$18.25 commensurate with experience.
Develop a monthly age-appropriate activity calendar including various group and individual recreational programs based on registrant preferences capabilities and needs.
Compile a list of supplies needed each month to conduct the activities and submits the list to the Program Director for approval.
Complete the initial and annual recreational assessment for each registrant per schedule.
Incorporate the registrants individual activity plan into their comprehensive plan of care.
Document daily activity program attendance for each individual registrant utilizing the monthly form.
At least quarterly or as needed review with the registrant his/her activity program participation and revise the care plan as necessary.
Maintain a safe environment for registrants when out of the facility on an outing.
Attends care plan meetings staff meetings and facility inservicing as directed by the Program Director.
Perform duties according to the highest ethical and moral standards and in adherence with the facilitys corporate compliance plan and all applicable governing laws. Adheres to the facilitys confidentiality statement in accessing and safeguarding protected health information (PHI) of registrants and employees as necessary to perform job duties.
Perform any other duties as requested by the Director and/or Administrator.
Qualifications
High School Diploma and one year of experience of recreational experience preferred.
Ability to plan and organize recreational programs.
Must have a sincere desire to work with the elderly and have an understanding of the social emotional physical and cognitive needs of the elderly and developmentally disabled.
Must have patience a cheerful disposition enthusiasm and be able to work with registrants based on their functioning status.
Must have a valid New York State drivers license.
Ability to follow written and verbal instructions as well as ability to meet established deadlines.
