DescriptionJob Title: Care Coordinator - Psychiatry Administration (Rivington) - Full Time - Day
- The Care Coordinator is responsible for coordinating care and guidance to medically complicated high-risk patients who are managing end-of-life care.
- Develops effective interpersonal relationships with patients and their families works collaboratively as an integral member of the interdisciplinary care team to develop and coordinate to ensure that patients receive appropriate clinical and social services.
- Utilizes internal and community resources
Responsibilities- Works with the interdisciplinary care team to develop a plan of care that addresses all clinical social and other needs that patients who have been diagnosed with ALS may need to ensure the highest quality of life for as long as possible. Collaborates with and coordinates all members of the care team and supports the patient and the patients family members.
- Identifies obstacles for patients that may inhibit their care plan such as child care and transportation. Works collaboratively with other members of the care team to ensure that all essential social services are met.
- Server as the primary liaison with the funder to facilitate communication and collaboration between the program and the organization. Advocates on behalf of patients enrolled in the program who are requesting financial or other support from the funding organization.
- Coordinates care with other members of the care team to overcome any identified barriers that inhibit patients medical care.
- Communicates with external vendors to facilitate adherence to the care plan such as transport companies childcare facilities durable medical equipment companies legal and housing services etc.
- Documents all patient encounters as appropriate in hospital documentation system and assists with the development of standardized reports on quality utilization processes and outcomes of the care coordination process.
- Educates patients regarding how to navigate throughout the health system.
- Partners with families and patient care team and community resources to provide well-coordinated timely compassionate exemplary interdisciplinary care.
- Serves as resource for community based education screening symposiums educational sessions lectures staff training etc.
- Performs other duties as required.
Qualifications- Bachelors degree required preferably in a health-related profession.
- Some experience in health care patient navigation or related field preferred.
- Bi-lingual individuals (English/Spanish) desirable.
