Position Summary
As an integral member of the Patient Education and Engagement Program the Coordinator helps create a culture of health literacy patient education and engagement across UHN hospitals through teaching and facilitation. The Coordinator works closely with all members of the Patient Education and Engagement team and in partnership with clinical teams and health professions teaching patient engagement care practices to build them into daily operations at UHN.
This role will specialize in capacity building for UHN staff in skills related to patient engagement including health literacy the UHN Patient Declaration of Values resource development patient education and engagement skills. The coordinator will work with clinical teams and units and the health professional/collaborative academic practice structure embedding patient education and engagement practices into health professions daily work.
Duties
- Plan organize and coordinate activities to support building the capacity of Team UHN to engage patients in their care
- Utilize strong knowledge of principles of health promotion and prevention self-management and self-care
- Work closely with the program and portfolio to identify shared education needs and opportunities
- Collaborate with team and all other clinical and non-clinical stakeholder to develop and implement research and/or quality improvement strategies and to disseminate results
- Facilitate teaching sessions including Orientation Teach-back and health literacy content
- Evaluate education delivered and partner with clinical teams on their own education
- Create new curricula as needed collaborating in the development implementing and evaluating strategies that support educational programming and projects across the site
- Participate in resource development and quality improvement initiatives
- Administer surveys and collect data and other duties consistent with the job classification as required
Qualifications :
- Bachelors degree in a health-related field or equivalent
- At minimum two (2) years of relevant practical experience
- Experience teaching or facilitating virtual and in-person educational sessions with healthcare professionals or other adult groups
- Knowledge of population health principles and the determinants of health as demonstrated through a combination of education and practical experience
- Knowledge of principles of effective health communication; adult education plain language patient engagement
- Experience planning implementing and evaluating education programs with diverse communities and community-based organizations
- Outstanding communication (verbal & written) customer service and listening skills that demonstrate compassion empathy and respect for colleagues patients and families
- Experience in a healthcare setting and working with interprofessional teams
- Ability to work in a self-directed manner
- Ability to identify connections across different hospitals and programs and meet multiple program needs simultaneously
- Ability to work effectively as a team member
- Familiarity with theories of health behavior patient engagement health literacy plain language adult learning theories preferred
- Fluency in a language other than English is an asset
