Duties
- Plan manage evaluate and coordinate comprehensive individual programs for patients with moderate to advanced dementia and their families within the context of an inter-professional practice model
- Education of patients family referring facilities and other community providers regarding care and behavioural support strategies of older adults with moderate to advanced dementia
- Use theoretical and empirical knowledge and the input of team members to plan and implement care for patients with moderate to advanced dementia and their families
- Provide occupational performance assessment recommendations to patients families caregivers and team to optimize occupational performance
- Prescribe appropriate seating and wheelchairs
- Collaborate with team members in an inter-professional environment providing patient-focused care and comprehensive discharge planning
- Communicate effectively with patients and families external agencies vendors and the community
- Participate in the clinical education of occupational therapy students
- Support the development and implementation of best practice models of occupational therapy
- Participate in Allied Health meetings and quality initiatives
- Provide evidence-based and patient-centered practice to patients seen
Additional Focus for This Role:
- Patient population includes individuals with moderate to advanced dementia who present with responsive behaviours including verbal and physical expressions of risk
- Developing implementing and monitoring individualized behaviour management strategies in collaboration with the inter-professional team
- Cognitive assessment and treatment as part of comprehensive care
- Active involvement in discharge planning aligned with a home first philosophy whenever appropriate
Qualifications :
- Masters of Science degree in Occupational Therapy or equivalent
- Current registration with the College of Occupational Therapists of Ontario (COTO)
- Canadian Association of Occupational Therapists membership preferred
- Previous experience in an acute care hospital environment preferred
- Excellent interpersonal time management and organizational skills
- Must be physically capable of performing the daily activities associated with patient care and occupational therapy treatment
- Demonstrated interest in working with older adults with cognitive and mental health challenges
- Experience in assessment and treatment of patients with dementia and associated responsive behaviours
- Demonstrated appropriate and effective written oral and nonverbal communication skills
- Computer literacy
- Authorization with the Assistive Devices Program (ADP) preferred; candidates without current authorization will be supported to pursue training/approval.
