Job Description
Shift and Status
Full time Monday to Friday 8:30 a.m. to 5 p.m. No weekends
Hybrid: field visits with patients in the St. Joseph surrounding areas.
Corewell Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six month or less serving individuals in their home nursing facilities and in the hospital. About Corewell Health South Lakeland Hospitals
The health system comprises threehospitals an outpatient surgery center a regional cancer center rehabilitation centers two long-term care residences home care and hospice services and 34 affiliate physician practice locations. We provide both inpatient and outpatientmedical servicesthroughout Southwest Michigan.
Scope of Work
In collaboration with the entire interdisciplinary team assess plan implement coordinate monitor and evaluate the strategies and services required to meet a patient/familys end-of-life care needs. An outcome-based plan of care is created and continually adjusted through utilization of the nursing process relationship centered care principles and holistic philosophy assuring the changing needs of the patient/family are met while facilitating a symptom-free peaceful sacred death.
Qualifications:
Required
Associates Degree Nursing
Registered Nurse (RN) in State of Michigan upon hire
Drivers License in State of Michigan upon hire
Preferred
Bachelors Degree Nursing
1 year of relevant experience of acute care emergency or rehab facility experience
