Minimum Qualifications:
Baccalaureate Degree ADN Degree or Diploma RN. Major: Registered Nurse
RN with a minimum of one (1) year of RN experience.
Current BLS certification.
Valid Arkansas Registered Nursing License
Preferred Qualifications:
ACLS certification or obtain certification within one year of hire date.
Knowledge or experience in PCMH desired.
Experience in an ambulatory care setting.

Responsibilities:
Provides education to patients and their families about disease prevention wellness maintenance health promotion and management of their condition. Help patients and families understand care plans and what to do between visits.
Arranges follow-up appointments ensure continuity of care communicate with providers and clinic staff track patient progress over time.
Handles prescription refill requests administer medications (as per physician orders and clinic policy) monitor for drug interactions or side effects ensure patient understanding of medications.
Performs clinical assessments identify patientspecific health needs develop individualized care plans in collaboration with providers and implement those plans.
Develops specialty knowledge in the clinics population (e.g. womens health issues obstetrics gynecology) act as a resource to other clinical staff and providers.
Measures patient outcomes assess how patients are doing relative to goals make modifications to care plan or interventions when needed.
Works closely with physicians nurse practitioners medical assistants and other clinic staff to deliver coordinated patientcentered care. Communicate clearly about patient status concerns or changes.
May perform other duties as assigned.
