Description

Under the direction of Clinical Nutrition leadership (Senior Director Director Clinical Nutrition Manager) demonstrates advanced practice in clinical focus area providing quality nutrition care and education to patients and family members. The Clinical Nutrition Coordinator organizes projects and initiatives support goals of the designated ambulatory practice. Participates in quality assurance performance improvement activities and adheres department/hospital philosophies standards objectives and policies and procedures.



Responsibilities

1. Demonstrates advanced practice in clinical focus area. Exhibits advanced knowledge and skills seeking out professional development and educational opportunities to further expertise.

2. Evaluates the nutritional status of the patients by reviewing pertinent information obtained from the electronic medical record and the interdisciplinary healthcare team. Applies experience and knowledge of nutrition screening and assessment to provide medical nutrition therapy by developing and implementing individualized nutrition care plans for general and practice specific patient populations

3. Provides individualized counseling to patients and caregivers including management of food-drug interactions therapeutic diets and supplements.

4. Conducts nutrition-focused physical exams as indicated to determine risk and/or degree of malnutrition.

5. Documents all encounters in the electronic health record per Clinical Nutrition policy.

6. Completes necessary paperwork to support insurance billing/reimbursement process.

7. Completes and maintains administrative records including productivity annual competencies and other documentation of direct and non-direct patient care functions with timely submission to direct supervisor.

8. Functions as a vital part of the multidisciplinary team in designated practice actively engages in discussions regarding the nutrition care of patients in the practice. Participates in interdisciplinary rounds and care plan meetings as needed.

9. Coordinates projects and activities including nutrition related in-services interdepartmental nutrition education programs lectures community-focused programs and external speaking events.

10. Assists with care of patients on home nutrition support. Evaluates nutrition support prescription and communicates with the clinical care team to ensure patients needs are met. Assists with discharge planning and hospital intake for patients on home nutrition support.

11. Demonstrates leadership through the coordination and implementation of initiatives that promote nutrition services in the practice.

12. Coordinates quality assurance monitoring and reporting activities. Demonstrates willingness to offer outcomes-based resolutions that promote sustainable improvement. Supports interdisciplinary performance improvement goals of ambulatory practice.

13. Develops and updates nutrition education materials that meet the specialized needs of the target patient population.

14. Precepts dietetic interns and instructs them on the competencies required to complete their assigned rotation develops learning modules and completes student evaluations.

15. Participates in research activities for ambulatory practice.

16. Maintains a positive and professional demeanor and effectively communicates with patients visitors colleagues hospital staff and supervisors.



Qualifications

Education Requirements

Bachelors degree or higher from an accredited institution with completion of ACEND (Accreditation Council for Education in Nutrition & Dietetics)-accredited dietetic internship.

Experience Requirements

3 years of clinical experience

Licensing and Certification Requirements (if applicable)

Registered dietitian (RD) from the Commission on Dietetic Registration (CDR) required

Certified Dietitian Nutritionist (CDN) from the NYS Department of Education Office of the Professions required or process initiated upon hire.

Advanced Certification i.e. Certified Diabetes Educator (CDE) Certified Nutrition Support Clinician (CNSC) and/or Pediatric/Renal/Oncology Specialty encouraged but not required.
