Full Time Admission/Discharge/Transition RN needed in Acute Care!
This is a day shift position!
GENERAL OBJECTIVES:
- Supports organizational throughput by initiating admission process
- Collaborates with nurses physicians and other team members to identify patients who are high risk for readmission and implements appropriate interventions
- Assists primary nurses in providing patient education throughout stay and upon discharge
- Coordinates care with appropriate personnel for safe and efficient patient transitions to next level of care.
PRIMARY DUTIES AND RESPONSIBILITIES:
- Utilizes advanced assessment skills to complete admission documentation to include assessment and admission history medication reconciliation core measures and anticipated discharge needs advanced directive information.
- Works closely with case manager and nursing staff to identify and intervene for patients at high risk for readmission.
- Coordinates and delivers patient and family education and information needs
- Facilitates throughput by identifying pending and confirmed discharges; communicates with department facilitator and House Supervisor for direction for areas with priority for admission and discharge.
- Collaborates with nurses and providers to ensure accurate discharge medications and patient education
- Coordinates follow-up care with PCP & specialty care physicians; including scheduling follow-up appointments prior to DC and assisting with patient needs to attend appointment such as working with case manager to arrange transportation.
- Coordinates and maintains a telephone follow-up program for patients that are identified as high risk and discharged to home.
- Assists primary nurses with specialized educational needs as needed (e.g. wound care diabetes colostomy Lovenox home IV antibiotics). Keeps the patient education resource list up to date on the intranet.
- Monitors the impact of transitional care services on patient care delivery and provide written report quarterly.
- Monitors/audits standard of care for education and transitions
- Seeks out technology improvements for delivery of patient education. (Care notes Exit Care ect.)
- Manages regulatory and accreditation requirements regarding patient education and transitions in care.
- Works in RN Acute Care role
QUALIFICATIONS:
Education:
- Current Colorado RN License or ability to obtain.
- BSN preferred
- 1 year of RN experience is required; minimum of 3 years of clinical experience is preferred.
- Ability to work collaboratively with diverse medical staff.
- Must possess excellent communication and organization skills
SALARY RANGE
- $37.50to$56.23/hour (BSN)
- Entry salary dependent upon education skill set and experience.
- Valley View recognizes a 4% pay difference for RN-ADN BSN MSN
- $2.25/hour evening shift differential when working a minimum of 4 consecutive hours between 3:00pm-11:30pm.
- $4.00/hour night shift differential when working a minimum of 4 consecutive hours between 11:00pm - 7:30am.
- $4.25/hour weekend shift differential when working 5 or more continuous hours between 7pm on Friday and 7am on Monday.
